This Project Completion report form must be submitted within two months after your project has been completed. Please note that if you don't complete and return a satisfactory report you or your group will not be eligible for future Creative Communities Scheme funding.

Financial Report

PROJECT COSTS: Please give details of how the money was spent.
Write down all of your project costs. Include all items from the budget in your application. Complete as many rows as required. List each item (e.g. venue) and for each item list the budgeted cost (from your application e.g. $600), the actual cost (e.g.$400) and enter a reason if there is a difference in amounts (e.g. project moved to cheaper venue).
PROJECT INCOME: Write down all of your income. Include all items from your application budget. Include your Creative Communities Scheme grant.
For each item list the name of the item, the budgeted income from that item (from your original application), the actual income from the item and list a reason if there is a difference between the amounts.
Note: You may be required to provide receipts for this project. Please keep your receipts in a safe place for seven years.
